Terms of Application

1. Malaysian citizen or permanent resident
2. Ages 18 and above
3. Has a company registration by Companies Commission of Malaysia (SSM) and the applicant has a share in the company
4. A company controlled by a foreigner is required that the applicant must be Malaysian citizen or permanent resident status by having a minimum shareholding of 30% in the company or the total ownership of the Malaysian citizen or permanent resident amounting to not less than 30%
5. The applicant conducting the business within the premises of the Local Authorities (PBT) is required to have a valid Local Authority License.
6. The applicant is not a bankrupt

How to Apply

New Application or Renewal Application
Fill the form BORANG PERMOHONAN LESEN KELUARAN PERTANIAN (Borang LESEN2017) and send to the nearest FAMA Office at the applicant's location. This free form is available at FAMA Headquarters and all FAMA State or FAMA District Offices or download form through the FAMA website. The application guidelines are as follow:-

New License Application Guide

1. Form Borang LESEN2017 is filled in 2 copies (capital letters)
2. Attach 1 copy of the applicant's Identity Card (IC)
3. Attach 2 pieces applicant’s color foto passport sized
4. Attach 1 copy of information of Companies Commission of Malaysia (SSM)
5. Enclose RM200 for new license processing fees payable in cash / check / money order

License Renewal Application Guide

1. Form Borang LESEN2017 is filled in 1 copie (capital letters)
2. Attach a copy of IC, SSM is still valid and an original license card
3. Application for renewal of license is free of processing fee
