Terms And Conditions And Methods Of Application

• Fill in Certificate of Origin Form which can be obtained from FAMA office at RM10.00 per pad (contain 50 sheets of form).
• Produce completed Export Permit Application (PK 2 Form) and Import Declaration (K1).

Certificate Fee

• RM8.00 for every validated certificate in cash / cheque / postal order / cashier order
